The Hamilton Niagara Hadlimand Brant LHIN IHSP-2 supports the MOHLTC’s priorities to promote equitable access to health and health care for all Ontarians. Equity is not explicitly included in the LHIN’s mission, vision or priorities for patient flow, chronic disease prevention and management, mental health and addictions, and enablers for transformation. Ensuring equitable access to services was discussed in relation to many of these priorities.
The LHIN’s decision-making framework includes strategic fit, population health impact, system performance, system values and readiness domains and then identifies decision criteria within each. Equity is one criteria within system values.
The HNHB LHIN also produced a population health atlas; it identifed sub-population differences in health behaviours and status, and diverse and low-income populations facing health challenges. It also highlighted the social determinants of health as complex and inter-connected, the need to collaborate across sectors, the importance of community participation in shaping priorities, and the need for up-stream investment.
