Transforming Health Care: One Person at a Time
- Go to Champlain LHIN website
- See the Champlain LHIN 2010-2013 IHSP
- See a snapshot from Champlain LHIN’s previous IHSP
The IHSP aligns with the MOHLTC’s priorities to improve health and promote equitable access to health and health care for all Ontarians. However, there is no other explicit mention of “equity” or “equitable” access in the IHSP.
Focusing on Specific Populations
- Integration initiatives in the LHIN focus on several specific populations, including:
- People with diabetes or pre-diabetes
- People with mental health issues and/or problematic substance abuse
- People with complex health conditions
- People are often unable to get the help they need at the time they need it the most; for example, the LHIN needs
- more mental health and substance abuse services for Aboriginal youth
- better coordination of services for the Francophone population
Rationale for Focusing on Specific Populations
- These groups face significant barriers to accessing quality and coordinated care
- They rely on the health system to help maintain and improve their health
Mission
- To build a coordinated, integrated and accountable health system for people where and when they need it
