We know a great deal about broad determinants of health and health inequities, and exemplary work is underway to address the social determinants of health. However, there are limitations to our ability to fully reduce health inequity. Integrating existing services and supports at the city and neighbourhood and community level increases opportunity for children and youth to succeed. Building on what enables success, Supports for Success is focused on developing an integrated framework of community-based supports, in collaboration with community members, to ensure healthy lives of children and youth from birth to employment.
